Temporary Permission to Practice for Non-licensed Pharmacist Immunizers

Special Work Permit for Pharmacists

The Board has authorized the issuance of a Special Work Permit for Pharmacist to a non-licensed pharmacist immunizer working within the State of Louisiana.  For now a paper application form is available.  The applicant must identify an active and unrestricted pharmacist license from any state other than Louisiana as well as their NABP eProfile identification number.  No fee is required.  Staff will verify the active status of the license and an absence of disciplinary restrictions, issue the Special Work Permit, and then alert the applicant by email.  The permit is virtual in format, meaning no paper form exists; it is verifiable at the Board's website.  The permit will expire 90 days after the date of issuance; if the permit is still needed, the permit holder may email a request for an extension of the expiration date.  The link to the paper application form is avaiable in this article as well as on the Pharmacists page in the Forms & Applications section of the Board's website.  An online application will be available by January 2, 2021.
